Open Line: Request for Action

The Open Line Request for Action is available on-line to expedite resolutions to common road and water complaints. Complete the questions below and press submit. We will respond via email with an assigned tracking number to your request during the business week. It is extremely important we have a current email address for communication.

In completing the information boxes on the form, we require a name, phone number and email address. Please provide as much information as possible in each box. Select a type of complaint from the pull down menu. If the category is other, use the space provided for other description.

STOP SIGNS should be phoned in during business hours or reported to the 911 dispatcher after business hours/weekends.
