Access Services
Services provided directly by Aging and Long Term Care staff.
| Information and
Assistance (I&A) |

I&A
staff assist seniors and their caregivers by linking them to
available resources including:
|

Assesses the problems and capacities of individuals and
provides general information, support, and advocacy. |
 |
Provides prescription assistance access services
to any adult over the age of 18. |
 |
Provides dental access assistance to
adults 65 and older. |
 |
Offers outreach and education to the community
about Kitsap A/LTC services and networks with other providers to maintain an
updated resource directory for clients and staff. |

For elderly and functionally impaired
adults over age 18 at risk of institutionalization case management
staff: |

Assesses needs. |

Develops care plans. |

Authorizes and arranges services.

providers and follow up with re-assessments as needed. |

| Family Caregiver Support
Program (FCSP) |
 |
Family
Caregiver Support plans, coordinates and provides services
and support for unpaid family caregivers of adults with functional
disabilities. The program provides information and assistance,
training, respite care, limited legal and nutrition services and
counseling and other services to help support the family
caregiver in providing care to a loved one. |

A Caregiver Support Center is located in
the Silverdale Community Center in Silverdale, with
 |
a caregiver resource library with
internet access with links to caregiver resources, books, videos and periodicals available for loan, and
|
 |
individual counseling and
consultation on care giving and coping skills. |
 |
Caregiver Support is open for
walk-ins Monday through Friday from 11:00 a.m. to 2:00 p.m., or
by appointment. |
