Services funded by Kitsap A/LTC and subcontracted to community
providers.

Kitsap A/LTC contracts with
Chuckwagon
Senior Nutrition Services to provide home-delivered meal services in
our county. |
 |
Home-delivered meals are provided to
vulnerable older persons who are home-bound by reason of illness,
incapacitating disability, or who are otherwise isolated.
|
 |
This program intends to
 |
maintain or improve the health
status
of these individuals. |
 |
support their independence. |
 |
prevent institutionalization. |
 |
allow earlier discharge from
hospitals,
nursing homes, or other residential
care facilities.
|
|
 |
Each meal served contains at least
one-third of the current Recommended Dietary Allowance (RDA) as
established by the Food and Nutrition Board of the National Academy of
Sciences National Research Council. |

Kitsap A/LTC contracts with
Kitsap
Mental Health Services to provide both Older Adult Counseling and
Substance Abuse services.

Older Adult Counseling provides
outpatient and consultation/ education services designed to
 |
evaluate an older person’s need
for mental health intervention. |
 |
determine the type of intervention
needed. |
 |
provide appropriate treatment or
referral for treatment.
|
 |
disseminate information to help
older persons gain access to needed mental health and other
community services. |
 |
provide specialized training,
consultation and education to community agencies and
organizations to improve community services and increase
awareness of mental health issues.
|
|
 |
Chemical Dependency Counseling
provides
 |
specialized assessments for persons age 60 and above, which include
individualized treatment recommendations. |
 |
assistance in obtaining treatment
at whatever level available and the client is willing to accept.
|
 |
community planning efforts to
provide enhanced services for persons in need of substance abuse
services, both in and out-patient. |

COPES and Medicaid in-home Personal Care Services are provided by
contracted home care agencies and/or Individual Providers.

Aging Network Personal Care and Respite
personal care services are provided through contracted home care
agencies only. |

Personal Care Services helps an individual
to stay in his/her home and live as independently as
possible. |
 |
Services may include assistance with

eating |

dressing |
 |
toileting |
 |
ambulation |
 |
transferring |
 |
positioning |
 |
bathing and |
 |
self-medication.

 |
Services may also include adaptations to
the home, Personal Emergency Response Systems, and skilled nursing care
and caregiver training. |

Respite Care

caregivers of adults with functional disabilities from caregiving duties for short time

daily basis, including 24-hour care for several consecutive

| Title V Senior
Employment |

The Title V program is an employment and
training program that
 |
provides part-time paid
community service training opportunities. |
 |
fosters individual economic self-sufficiency through
subsidized training and subsequent placement into unsubsidized
jobs for Kitsap County residents who are age 55 and over and who
are in the greatest economic and social need. |

 |
Kitsap A/LTC currently contracts with
WorkSource of Kitsap County, a division of the Washington State
Employment Security Department to provide senior employment services.
