Accidental Death & Dismemberment Coverage for employees and dependents.  No approval needed.  Sign up anytime by completing the enrollment form and submitting to the Personnel Office.  (Employees may not be double-covered under this policy.)

Voluntary VTL Coverage

Voluntary Term Life (VTL) Brochure
Voluntary Term Life Coverage is available to employees and dependents.
There is a guarantee issue amount for new employees who sign up during their first 30 days of employment. 

SPECIAL GUARANTEE ISSUE OFFER! Current employees who did not take advantage of the guarantee issue when hired, have the opportunity for the guarantee issue amounts again.  An application form and beneficiary designation form must be completed, signed, and returned to Personnel prior to January 31, 2008. Spouse signature is required (back of application) if requesting spousal coverage. Rates are listed in the brochure. (Employees may not be double-covered under this policy.)

Guarantee Issue Amounts (No medical underwriting required):
Employee - $50,000
Spouse - $20,000
Children - $10,000
