REIMBURSEMENT REQUESTS

The Homeless Housing Grant Program and the Affordable Housing Grant Program share a single reimbursement request form:

This link takes you to our electronic submission system, powered by DocuSign. This form includes a Reimbursement Request Form and a Summary Statement of Expenses.

QUARTERLY REPORTS: AFFORDABLE HOUSING GRANT PROGRAM

Affordable Housing Grant Program grantees must submit a quarterly report for each quarter of their contract, regardless of whether there was any grant activity.  Quarterly reports are due the last day of the month following the end of the calendar quarter (April 30, July 31, October 31, January 31).  All Quarterly Reports are submitted electronically through DocuSign.

Important Note:  Before you click the link, you must have all the information you will need to submit the reimbursement request assembled.  Unfortunately, you can not save your work once you've started.  Every time you click the link a new report is sent whether you finish it or not.

AHGP Quarterly Progress Report

Submit the Final Progress Report at the conclusion of your Affordable Housing Grant instead of the 4th Quarter Report. Before clicking the link above, please be prepared to completely fill out your Final Report and submit it.

ð Remember, for all Homeless Housing Grant Program contracts you must also submit an HMIS Quarterly Report, pulled directly from HMIS and then emailed as an Excel file to kjewell@co.kitsap.wa.us.
