Invitation to Teachers - Submit by March 8, 2013

Please complete this short form to be included in the lottery for an opportunity to attend the National Public Works Week field trip at the Kitsap County Public Works Annex, 8600 Imperial Way in Bremerton, on May 21, 2013. Please note the following:

Teachers are responsible for transportation
This date falls within the MSP testing window. Please be sure your class is available before submitting your name for the lottery.
Due to space and time constraints we cannot guarantee that multiple classes from the same school will be able to attend.

	Yes! We would like to participate in the field trip to the Public Works Annex, on Tuesday, May 21, 2013.
	We're unable to participate in the field trip but we would like to learn more about having Public Works visit our classroom.
	Not interested but please consider my classroom for next year's event.
